Job Description
Summary:
The Oncology Nurse Navigator RN is a professional registered nurse with oncology-specific clinical knowledge who offers individualized assistance to patients, families, and caregivers to help overcome healthcare system barriers. Provides education and resources to facilitate informed decision making and timely access to quality health and psychosocial care throughout all phases of the cancer continuum. The Oncology Nurse Navigator RN Serves to lead the other team members who may be assisting with the navigation process: community health worker, financial counselor, navigator, social worker, etc.
Responsibilities:
Collaborates with the Administrator, Oncology Program to develop care pathways and guidelines to facilitate care and referrals for cancer patients; Works with the Medical Director, Oncology, to involve referring physicians and oncologists in developing care pathways and guidelines for referral; Works collaboratively with key stakeholders within YRMC and the community, as well as with academic partner to achieve excellent care coordination; Works with inpatient and outpatient clinical staff to facilitate timely and high quality care for cancer patients; Evaluates newly-diagnosed and ongoing cancer patients to determine services needed and makes referrals as necessary; Facilitates appointments for diagnostic testing, treatment and follow-up; Provides business staff with the clinical information necessary to facilitate the process of obtaining authorization; Interfaces with payers at the request of business staff to directly provide clinical information as needed; Works collaboratively to ensure that staff provides necessary teaching to patients; Collaborates with community resources to provide seamless transition of care from the cancer center to hospital, other ambulatory care settings and back into the community; Facilitates staff's coordination of all patient activities, ensuring that arrangements are made in a timely and cost effective manner; Develops and maintains strong working relationships with referring and oncology physicians to facilitate communication and assure that patients are seen in a timely manner.
Provides human resource management for area of responsibility including ensuring appropriate staff are available to meet the needs of the unit, providing input into the selection process, setting clear work expectations and coaching employees as needed for success; Participates in the selection, orientation, supervision, continuing education, and evaluation of personnel; Adjusts staffing and work schedules to provide adequate coverage as related to workload.
Treats guests, patients, physicians and other employees with care, courtesy and respect; Presents a positive image of the Yuma Regional Medical Center Oncology Program; Role models positive, professional relationships and communication by establishing and maintaining rapport with patients, families, community, members of the healthcare team, health care agencies and providers; Responds quickly and appropriately to situations, taking responsibility to solve the problem at hand; Collaboratively develops initiatives to enhance the patient experience throughout the continuum of care; Demonstrates an ability to plan, organize, coordinate, prioritize, balance workload and follow through to achieve institutional and individual goals and objectives; Participates on Tumor Boards and other educational meetings (as required); Collaborates with internal departments around improving patient care and hospital-physician relationships; Promotes teamwork and communication with other caregivers to meet patient/family needs and to achieve desired therapeutic and financial outcomes; Maintains successful patient and physician satisfaction within the oncology service line, as demonstrated by satisfaction ratings; Demonstrates a communication style that serves to reduce and resolve conflicts.
Collects and collates patient data to measure quality and effectiveness of process and outcomes of care; Actively participates on appropriate teams, providing insight into care delivery processes and opportunities for improvement; Maintains and expands knowledge of current trends and developments in the healthcare and finance fields; Exhibits a commitment to operational management and completing tasks through personal initiative and persistence; Demonstrates use of good judgment and sound decision-making; Contributes new ideas and new ways of thinking to improve departmental performance and services; Works effectively with the Administrator, Oncology Services, the Medical Director Oncology services and other hospital departmental staff to assure a high-quality patient care environment; Ensures that staff and physicians utilize appropriate pathways or guidelines, and that data is collected appropriately; Maintains knowledge of oncology care coordination and navigation programs and regularly participates in educational opportunities; Develops mechanisms to assist business staff members in obtaining and interpreting clinical data and information; Conducts formal training programs and develops materials to increase other staff's knowledge of care coordination resources and techniques; Implements an effective and on-going program to monitor, evaluate and improve the quality of services delivered.

- BLS
- RN Licensure
- 3+ YOE

Work Schedule:8-hours, Days, Monday through Friday
