Summary

Under the direction of the Director Ambulatory Operations - FMC Primary Care, the Population Health Coordinator will be responsible for collecting, analyzing, and interpreting data to support organizational population health initiatives. This position partners with operational leaders and providers to assist in the implementation and training of population health initiatives. The Population Health Coordinator monitors, analyzes, and validates claims and clinical data to support the healthcare team in proactively managing identified patients. 

Responsibilities

Reviews and analyzes population and patient data related to clinical care and outcomes, to evaluate performance, reveal trends, and identify opportunities. Create reports and dashboards that effectively communicate performance to operational leaders and providers Implement and train healthcare team on population health initiatives Other duties as assigned

 

Other Information

REQUIRED: 2 years of healthcare related experience. Strong analytical skills. Computer proficiency (must be able to pass computer proficiency assessment). PREFERRED: Experience with EPIC Electronic Health Records or similar electronic record system. Experience with population health quality measures to include HEDIS

Education

Essential:
* HS DIPLOMA/GED

Nonessential:
* BACHELOR'S DEGREE




Work Schedule: 8-hours, Days, Monday through Friday
