Work as a non-Physician provider in a team with medical professional such a primary care physician, nurse practitioners, and clinical staff to provide quality patient care. Provide training in innovative, evidence-based health interventions that improve quality, improve patient-centered care, and reduce costs. Develop and establish behavioral aspects of integrated care in YRMC Family Medicine Center. Provide recommendations about patients and families to assist them and the health care team in coping with patient's hospitalization, illness, diagnosis, treatment, and/or life situation, including emotional, mental, and substance abuse disorders. This role will also work with the Department of Graduate Medical Education to assist in many layers of resident and fellow education as directed by the Designated Institutional Official (DIO) and Chief Academic Officer.

Minimum Requirements:

· Master’s Degree in Social work or Human Service related field

· LCSW (Arizona Board of Behavioral Health Examiners)

· Fingerprint Clearance Card (Arizona Department of Public Safety)

· Experience in outpatient counseling and patient assessments/ treatment plans

Preferred requirements:

· Experience with EPIC Electronic Health Record or similar electronic record systems

Work Schedule: 8-hours, Days
