Summary:

The Discharge Case Manager provides and facilitates patient discharge planning in collaboration with case managers, social workers, nursing and providers. This individual assists with organizing services across provider lines for an optima patient outcome and continuity of care.

Responsibilities:

Works as a liaison with patient families/caregivers and the care team to promote effective communication during the discharge process.

Identifies barriers to discharge, works to mitigate those identified barriers prior to day of discharge.

Ensure and verifies orders are placed for equipment to ensure safe discharge

Communicates discharge needs to the next level of care (i.e., Homecare, Long-term care, home caregiver).

Communicates to the patient/caregivers hospital follow-up/specialty appointments.

Verifies submission of new prescriptions placed at patient pharmacy and verifies the arrangement for discharge transportation.

Communicates to primary nurse when patient is ready (all orders placed and communicated, medication have been ordered, transportation arranged) for discharge to the next level of care.

Communicates with patient and caregivers.

Identifies patients at high risk for readmission and arrange for post discharge communication to ensure continued understanding of discharge, with input of Case Managers.

Other duties as assigned. 


Credentials:
Essential:
* BASIC LIFE SUPPORT (BLS)
* REGISTERED NURSE (RN)

Education:
Essential:
* ASSOCIATE'S DEGREE

Other information:

Minimum Education Required:

Associates Degree

Licensure required Registered Nurse with Arizona

BLS (AHA)

Three + years of RN experience, Case Management experience preferred.
