RN Case Manager II-Days

Summary:

The Case Manager II provides utilization management on all admitted patients; facilitates discharge planning, assists in coordination of patient transfers to acute hospitals and psych facilities; provides supervision for Case Management Assistants.

Responsibilities:

Completes discharge planning, meds/orders for assigned patients. Completes documentation in patient's EMR in timely manner. Completes biopsychosocial assessments within 48 hours on all assigned patients. Completes referral forms/applications/logs within Case Management department. Provides training for newly hired RN Case Managers within the Case Management department. Assists with process improvement projects. Provides supervision to Case Management Assistants. Other duties as assigned.

REQUIRED: 3+ years related experience

REQUIRED: Associates in Nursing-State Board of Nursing- AZ-
